If my divorce has a final decree from the state of Virginia, may I turn in a form Ex Parte Application for Restoration of Former Name After Entry of Judgment and Order in the state of California to have my name changed back to my maiden name? The form asks for a case number. Can California use a case number from Virginia?

No. You cannot use California forms or California courts for a VA matter. You have to do it in VA the VA way.
